Is the SMS75 only for sidemount, or can I use it back-mounted?

Both - cam slots take a single rear-mounted tank and the 11-in grommets accept banded twins, so it doubles as a crossover rig.

Does it have integrated weight pockets?

No - standard for sidemount systems, ballast is managed via the cylinders themselves plus optional belt/ankle weights.

What comes in the box?
